Center for the Aging Brain Selected for Innovative Population Health Management Program

The NYU Hartford Institute for Geriatric Nursing has selected the Montefiore Einstein Center for the Aging Brain, as well as five Montefiore Medical Group patient-centered medical homes, for implementation of an innovative population health management program to improve primary care services for older adults in the Bronx.

Funded by a three-year, $2.55 million Health Resources and Services Administration (HRSA) grant through the Geriatrics Workforce Enhancement Program (GWEP), the program will help Montefiore providers better integrate geriatric medicine into primary care  to maximize patient and family engagement, manage chronic disease, and promote optimal function in older adults. NYU and Montefiore will partner with Regional Aid for Interim Needs, Inc. (R.A.I.N.) to further integrate community resources into the care of older adults in the Bronx. The grant also provides increased geriatrics education for primary care providers, residents, and nursing staff focusing on key geriatrics topics including dementia screening and care of patients with Alzheimer’s disease.

Dr. Tara Cortes, PhD, RN, FAAN, professor of geriatric nursing and executive director of the Hartford Institute for Geriatric Nursing, is principal investigator of the study. Montefiore Einstein site leaders are Dr. Amy Ehrlich, Professor of Clinical Medicine (Geriatrics) and Dr. Jessica Zwerling, Assistant Professor of Neurology and Pediatrics (Neurology). Dr. Joe Verghese, chief of the Integrated Divisions of Cognitive & Motor Aging (Neurology) and Geriatrics (Medicine), director of Montefiore Einstein Center for the Aging Brain, and professor of neurology and of medicine (Geriatrics) at Einstein, is senior mentor.

Drs. Joe Verghese and Nir Barzilai have teamed up with Drs. Gil Atzmon and Roee Holtzer to identify interventions to keep us strong and agile into old age.


Dr. Joe Verghese's research was featured in a National Geographic cover story on the potential genetic roots of longevity.

Bogaisky's Readmission Study Receives AAMC Honorable Mention
Research conducted by Dr. Michael Bogaisky initiated a unique collaboration to identify and resolve the issues leading to high hospital readmission among the skilled nursing facility patient population.
