Patient Population and the Community Health Center

The home for ambulatory training for our PCSIM residents is the Comprehensive Health Care Center (CHCC). CHCC was founded in the 1960s near the beginning of the national community health center movement. It is a Federally Qualified Health Center (FQHC) which is managed via a partnership between Montefiore Health System and the Bronx Community Health Network. We serve patients no matter their insurance status or eligibility. Attending physicians and residents practice medicine alongside each other and care for the same patient population.

The building houses multiple services and specialties – adult medicine, pediatrics, OB/GYN, dental services, dermatology, and behavioral health. Since 2014, CHCC has maintained certification as a patient centered medical home. In keeping with the medical home mission, the adult medicine practice works within an interdisciplinary team framework. Our team members include – nurses, front desk staff, physicians, community health workers, social workers, doctors of pharmacology (with specializations in HIV care, treatment of opioid use disorders, and treatment of resistant hypertension), diabetes educators, insurance representatives and health educators. We aim to train our residents in the skills necessary to practice high quality team based care, participate in quality improvement and practice the skills of panel management.

CHCC is located in the South Bronx on 161st Street, a key corridor between the Morrisania and Mott Haven neighborhoods. The surrounding community has faced long-standing challenges, including housing barriers, environmental risks, and economic hardships. Medical care and education at CHCC focus on addressing these factors to improve health outcomes and patient care.
