Helping young adults with type 1 diabetes make the transition to adult care

El programa de Apoyo a Adultos Emergentes con Diabetes, parte del Instituto Fleischer de Diabetes y Metabolismo en Montefiore y Einstein, es un programa integral dedicado al cuidado de adultos jóvenes, de 18 a 35 años de edad, que viven con diabetes tipo I (DT1).

We provide young adults with cutting edge medicine and access to the latest diabetes technologies in addition to an array of events and programming that address the struggles and successes of living each day with type 1 diabetes.

At SEAD, you’ll find a multidisciplinary team offering comprehensive and compassionate care designed for young adults with T1D. The program includes support that addresses not only medical and nutritional needs, but also the psychosocial and developmental needs unique to this time in addition to peer support. We have physicians, nurse practitioners, a dietician, psychologist, technology navigator, and a social needs coordinator. With SEAD, young adults with T1D will find the individualized support and comfort they need to work toward positive, long-term outcomes in quality of life, and improved health.
