Professional Interests

Farrukh N Jafri, MD, MS-HPEd, FACEP, is the Medical Director of WPH Cares, a department dedicated to providing data-driven, innovative solutions to ensure patients have access to services they need. In this role, he has designed and operationalized a virtual health platform including a community paramedicine program, a clinical outreach program for every hospital discharge, a tele-clinical pharmacist and remote patient monitoring program as well as a virtual acute care clinic for complex care patients.

In his prior role, Dr. Jafri served as the Assistant Director of Education and Simulation and Co-Medical Director of the Armonk Urgent Care Center.  He built a simulation center and led a multi-hospital quality improvement initiative focused on COVID airway management, coached interprofessional teams in Pediatric and Adult inpatient units along with the Cardiac Catheterization Lab and the Departments of Intensive care, Emergency Medicine and Anesthesiology on Crisis Resource Management. He has designed medical virtual reality programs including an IDSA-recognized Personal Protective Equipment (PPE) training program. Dr. Jafri has been awarded ”Healthcare Hero” by Westchester Magazine, “Excellence in Healthcare” by the United Hospital Fund, “Rising Stars: 40 under 40” by the Business Council of Westchester and the “Medical Service Award” by the Children’s Dream Foundation. He has served as a research mentor to High School, College and Medical School students and has taught workshops at NYU, Mount Sinai, Henry Ford and Harvard University.

Dr. Jafri has a BA from New York University, obtained his MD from the Renaissance School of Medicine in Stony Brook and completed his residency training in Emergency Medicine at Mount Sinai St. Luke's-Roosevelt Hospital where he also served as Chief Resident. He has completed his Masters of Science in Health Professions Education at the Massachusetts General Hospital Institute of Health Professions and a fellowship at the Montefiore Center of Performance Improvement and is currently enrolled in a PhD at Massachusetts General Hospital Institute of Health Professions studying the impact of digital health for patient safety, quality and performance improvement.
