Professional Interests

Dr. Katherine E. Di Palo, PharmD, MBA, MS joined Montefiore Medical Center in 2015 as a board certified ambulatory care and geriatric clinical pharmacy specialist. In 2018, she was appointed Clinical Program Manager of the Hospital Readmissions Reduction Program. In this role, she worked with system leadership to implement a comprehensive transitional care model for heart failure which led to significant performance improvement in key value-based programs. Dr. Di Palo is currently the Senior Director of Transitional Care Excellence and an Assistant Professor of Medicine within the Division of Hospital Medicine.

Dr. Di Palo completed her Doctor of Pharmacy from the University of Buffalo, her Masters of Business Administration from Cornell University, and her Masters of Science in Health Care Policy and Research from Cornell University. She is a Fellow of the American Heart Association and Heart Failure Society of America.
