Clinical Rotations, Electives and Call Schedule

Required ICU and Elective ICU Rotations

As specified by the ACGME, there will be at least 9 months of ICU rotations. Our fellow will do 6-8 months of SICU rotations. One month will be a required Cardiac ICU rotation at Montefiore Medical Center. The Fellow can choose from multiple elective options including burn ICU, MICU, pre-hospital and anesthesia. Two weeks of intensive critical care echocardiography is incorporated into the curriculum, leading to echo board eligibility after requirements are met. This includes resuscitative transesophageal as well as transthoracic approaches.

Trauma Rotations

The Fellow will rotate on the trauma service for 2 months. The Fellow will collaboratively lead the trauma service with the PGY4 trauma chief resident while on rotation. As the year progresses, the expectation is that the Fellow would effectively act in a junior attending capacity to support the residents—taking them through operative cases (with attending supervision), support during trauma resuscitations, and provide teaching during rounds, didactics and simulations.

Other Elective Opportunities

As basic echocardiography skills become increasingly important to the surgical intensivist, we want to ensure that our Fellow graduates with some competency in Basic Critical Care Echocardiography. Time will be arranged for the Fellow to spend time with cardiology faculty and the echocardiography technicians along with regular didactic and practicums from the SICU faculty who used echocardiography regularly.

The Fellow will also have the opportunity to work with anesthesia faculty to practice their airway skills and obtain the ACGME required number of intubations.

Call Schedule and Vacation

The Fellow will take in-house call approximately once per week with some flexibility to suit the Fellow’s priorities and personal schedule. Call responsibilities include managing acute overnight issues in the SICU and responding to high level trauma activations, especially those that require operative intervention. While on the trauma service, call responsibilities are solely those pertaining to the trauma service. As the year progresses, graduated addition of responsibilities will lead the Fellow to effectively act in the capacity of a junior attending, with attending supervision in-house as backup.

The Fellow will have 4 weeks vacation.
