<p>Yaroslav (Jacob) Gelfand, MD, is an attending physician and Assistant Professor at Montefiore-Einstein. Dr. Gelfand&rsquo;s clinical specialty is in the use of minimally invasive approaches to diagnose and treat a wide variety of spine pathologies and problems. With extensive training in both minimally invasive and complex spine surgery, Dr. Gelfand offers the latest treatment options to patients, using cutting-edge technology including intraoperative image guidance, fluoroscopy, endoscopy and pre-operative computer planning.</p><p>After completing his Bachelor of Science in biomedical engineering at University of California, Davis, Dr. Gelfand studied his Doctor of Medicine at Weill Cornell Medical College, earning the degree in 2013. His time at Weill Cornell included a yearlong Howard Hughes Medical Fellowship at the institution&rsquo;s neurobiology and behavioral science lab. From 2013 to 2018, Dr. Gelfand pursued a residency in neurosurgery at Albert Einstein College of Medicine. He then completed a research fellowship at Columbia University as well as a nonclinical research fellowship with Globus Medical's Musculoskeletal Education and Research Center in 2019, before returning to be chief resident at Einstein&rsquo;s Department of Neurological Surgery from 2019 to 2020. In 2021, Dr. Gelfand completed his yearlong minimally invasive and complex spine surgery fellowship at the University of California&rsquo;s Los Angeles Spine Center.</p><p>Dr. Gelfand&rsquo;s research focuses on the latest spine technology, surgical techniques and patient outcomes. He has authored and co-authored over 35 peer-reviewed publications and book chapters along with many more abstracts and presentations. His research efforts have been presented at numerous national and international conferences and have been recognized with several distinguished awards. He is also a reviewer for Global Spine Journal, Clinical Biomechanics and Spine Journal.</p><p>Dr. Gelfand is a member of the Congress of Neurological Surgeons and the American Association of Neurological Surgeons.</p>

<p>Christina H. Fang, MD, is an attending physician and Assistant Professor, Department of Otorhinolaryngology at Montefiore Einstein. Fellowship-trained in rhinology and endoscopic skull base surgery, Dr. Fang&rsquo;s primary clinical interests are functional and advanced endoscopic sinus surgery, septum/turbinate surgery, sinus cancer, epistaxis, nasal valve surgery, cerebrospinal fluid leaks and minimally invasive endoscopic skull base surgery.</p><p>After earning her Bachelor of Science in biology from The College of New Jersey in 2011, Dr. Fang continued her education at Rutgers New Jersey Medical School, earning her Doctor of Medicine in 2015. Her postdoctoral training began at Einstein with a residency in otolaryngology&ndash;head & neck surgery, which she completed in 2020, acting as administrative chief resident in her final year. She was also named Chief Resident of the Year. She then pursued a yearlong fellowship in advanced rhinology and endoscopic skull base surgery at Rutgers New Jersey Medical School, which she completed in 2021.</p><p>Dr. Fang&rsquo;s research interests include evaluating evidence-based practices in rhinology and disparity in otolaryngology. She has published more than 60 peer-reviewed articles and book chapters in otolaryngology, rhinology and endoscopic skull base surgery. She has shared her work through several national scientific presentations, peer-reviewed journal articles, book chapters and poster presentations.</p><p>Dr. Fang is board certified by the American Board of Otolaryngology and is a member of the American Academy of Otolaryngology&ndash;Head and Neck Surgery, the American Rhinologic Society and the North American Skull Base Society. </p>

<p><strong>Clinical Interests: Epilepsy, Trigeminal neuralgia, Parkinson Disease, and Brain tumors</strong>.<br /> Dr. Eskandar specializes in the surgical diagnosis and treatment of epilepsy in both children and adults.&nbsp;&nbsp; He is a world-leader in this field, and has over 15 years of experience in utilizing the most current techniques. &nbsp;These techniques include keyhole surgery (minimal incision), stereotactic electro-encephalography (SEEG), minimally invasive foramen-ovale electrodes, vagal nerve stimulation, responsive neuro-stimulation (RNS or Neuropace), and laser ablation of epileptic areas. &nbsp;&nbsp;</p>

<p>Dr. Eskandar has vast experience in using deep brain stimulation (DBS) for the treatment of Parkinson Disease, Dystonia, Essential Tremor, and severe Obsessive-Compulsive Disorder.&nbsp;&nbsp; He employs different methods for surgery including awake-surgery with micro-electrode recordings, frameless surgery, and surgery under anesthesia using real-time imaging.</p>
<p>Finally, Dr. Eskandar treats all types of brain tumors including meningiomas, gliomas, low grade tumors, and metastatic brain tumors.&nbsp; He specializes in the use of advanced brain-imaging and brain-mapping techniques to minimize the risk of injury and to maximize tumor resection.&nbsp; Dr. Eskandar is a pioneer in brain mapping and has published many seminal papers on this topic.</p>

<p>The Eskandar lab has made numerous important scientific contributions.&nbsp; For example, one recent study, published in <em>Nature</em>, found that a part of the brain called the Cingulate Cortex plays an important role in adapting to varying degrees of cognitive difficulty.&nbsp; Another recent paper in <em>Nature Neuroscience,</em> found that delivering micro-stimulation in one part of the basal ganglia, the caudate nucleus, significantly increases the rate of learning beyond baseline rates.&nbsp; These findings suggest that the caudate plays a critical role in learning, and that learning can be enhanced to promote recovery after traumatic brain injury or stroke.&nbsp;</p>
<p><strong>Background:<br /> </strong>Dr. Eskandar received a Bachelor of Arts degree in chemistry from the University of Nebraska. He earned a medical degree at the University of Southern California, Los Angeles, and a master of business administration degree at the Sloan School of Management at the Massachusetts Institute of Technology. He was a neurological surgery resident at Massachusetts General Hospital in Boston, MA, and a neurophysiology fellow at Harvard Medical School.&nbsp;He previously, held the Charles Anthony Pappas endowed chair of Neurosurgery at Harvard Medical School where he also served as Professor of Neurosciences.&nbsp;</p>
<p>Dr. Eskandar is board-certified by the American Board of Neurological Surgery. <p>Emad N. Eskandar, MD, specializes in the surgical diagnosis and treatment of epilepsy in both children and adults. <p>Celina Crisman , MD, MPH, is an attending physician and Assistant Professor, Neurological Surgery at Montefiore Einstein. Dr. Crisman specializes in neurosurgical oncology and focuses on the diagnosis and surgical treatment of cranial lesions in adult patients. Through a dedicated fellowship, she received specialized training in surgical approaches to benign and malignant cranial tumors and enjoys applying the most advanced techniques and surgical adjuncts to each case. Dr. Crisman performs awake tumor resection when indicated and works closely with a multidisciplinary team to maximize resection of tumors in eloquent regions. She also cares for patients with general neurosurgical needs ranging from hydrocephalus, chiari malformation and herniated discs, as well as traumatic bleeds.</p><p>Dr. Crisman obtained her Bachelor of Science in biology and biology and society from Cornell University, and earned her Doctor of Medicine from Columbia University Valegos College of Physicians and Surgeons. She completed her residency in neurological surgery at Rutgers University and was a neurosurgical oncology fellow at Memorial Sloan Kettering. Dr. Crisman also earned a Masters in Public Health from the Harvard Chan School of Public Health.</p>

<p>Acute stroke, brain aneurysms, tumors and arteriovenous malformations (AVMs), spinal AVMs and carotid-cavernous fistulas (CCF), dural arteriovenous fistulas, vein of Galen malformations, vascular malformations and vasospasm.</p><p>Treatment methods include:</p><ul><li>Coil embolization of aneurysms</li><li>Transarterial embolization of AVMs and adrenal venous sampling (AVFs)</li><li>Carotid angioplasty and stenting</li><li>Intracranial stents for aneurysms and strokes</li><li>Transarterial, transvenous and direct puncture embolization of congenital AVMs and congenital and acquired AVFs</li></ul>

Research Focus

Past research has focused on the safety and efficacy of detachable micro-catheters for pediatric AVMs and the development of other minimally invasive technology for treating cerebrovascular disease. Dr. Altschul is currently involved in the ESCAPE trial evaluating endovascular stroke intervention at Montefiore.

<p>David Altschul, MD&mdash;a graduate of SUNY Downstate College of Medicine in Brooklyn, New York completed a general surgery internship, neurological surgery residency (he served as chief resident) and a fellowship in Endovascular Neurosurgery at Montefiore.</p><p>He also completed an endovascular neurosurgery fellowship at St. Luke's-Roosevelt Hospital, New York City.</p><p>Dr. Altschul is one of only a few surgeons in the region trained in both endovascular (interventional) and open neurological surgery for adult and pediatric patients.</p>

His clinical focus is on comprehensive treatments for adult patients with benign and malignant brain tumors. Dr. Agarwal specializes in the treatment of patients with primary brain tumors such as gliomas, meningiomas and pituitary tumors, as well as skull base and metastatic brain tumors. In addition, he treats cerebrovascular disease, including intracranial aneurysms and arteriovenous malformations.</p><p>Dr. Agarwal received his Bachelor of Science in Electrical and Biomedical Engineering in 2002 from the University of California, Los Angeles. In 2010, he received his Doctor of Medicine from Chicago Medical School, where he graduated with honors. Dr. Agarwal completed his residency in Neurosurgery at Duke University Medical Center in 2016 and a fellowship in Minimally Invasive and Endoscopic Skull Base Neurosurgery at the Center for MIS Neurosurgery in Sydney, Australia. In 2017, Dr. Agarwal completed a fellowship in Open Skull Base Neurosurgery at the Mayo Clinic, and in 2018 completed an Open Cerebrovascular fellowship at Emory University Hospital in Atlanta, Georgia.</p><p>Building on his clinical focus, Dr. Agarwal's research focuses on the genetics and proteomics of brain tumors. He has also published two textbooks, with another in process, on complex and combined skull base approaches with Cambridge University Press and Elsevior Publishing Company.</p><p>Dr. Agarwal is a member of the Alpha Omega Alpha Honor Medical Society, the North American Skull Base Society, the Congress of Neurological Surgeons and the American Association of Neurological Surgeons.</p>
