<p><strong style="text-align: justify;">Targeting signal transduction in hematologic malignancies</strong><span style="text-align: justify;">:&nbsp;<br /></span><span style="text-align: justify;">Cytokines play important roles in the regulation of normal hematopoiesis and a balance between the actions of hematopoietic growth factors and myelosuppressive factors is required for optimal production of different hematopoietic cell lineages. The myelodysplastic syndromes (MDS) are collections of heterogeneous hematologic diseases characterized by refractory cytopenias due to ineffective hematopoiesis. These preleukemic disorders are common causes of anemia in the elderly and are rapidly increasing in incidence. We have also demonstrated that the TGF-beta superfamily/smad, IRAK and MAP kinase pathways is overactivated in MDS. Our work has led to clinical trials with Luspatercept, Galunisertib, CA4948, ARRY-614 and various other promising agents in MDS. We are trying to study the molecular mechanisms that lead to the activation of these pathways in MDS and are using small molecule inhibitors in mouse models to target these pathways.&nbsp;</span></p>
<p style="text-align: justify;"><strong>Targeting aberrant stem and progenitor cells in myelodysplastic neoplasms:&nbsp;<br /></strong>We have demonstrated that aberrant stem cells persist during morphological remissions and are involved in relapse. We have identified novel targets against MDS/AML stem cells that are being tested in various studies. We are also conducting sequencing/epigenetic analysis of highly purified human stem cells and evaluating the role of these alterations in transformation of MDS to AML.&nbsp;</p>
<p style="text-align: justify;"><strong>Epigenomic analysis of tumors</strong>: <br />We are using high throughput assays to analyze DNA methylation across the genome and have optimized these assays to work with low amounts of DNA. We have successfully used these assays in conducting an integrative analysis of epigenetic and genetic alterations in various tumor models, including MDS, esophageal cancer, pancreatic cancer and renal cell cancer.</p>
<p style="text-align: justify;"><strong>Clinical studies in Myelodysplastic syndromes</strong>: <br />We have a &ldquo;center of excellence&rdquo; clinic for patients with Myelodysplatic syndromes. We offer a variety of clinical trials for these patients.</p>

<p>Amit K. Verma, MD, is Interim Chair, Department of Oncology, Associate Director of Translational Science, Montefiore Einstein Comprehensive Cancer Center, Professor, Medical Oncology and Professor, Developmental & Molecular Biology, Albert Einstein College of Medicine.</p><p>Dr. Verma earned his Doctor of Medicine at the All India Institute of Medical Sciences in 1995 and finished his training in internal medicine, hematology oncology and stem cell transplantation at the University of Illinois and Northwestern University in 2003.</p><p>Dr. Verma conducts clinical studies for patients with MDS and leukemia and offers a variety of clinical trials for these patients. He conducts laboratory research aimed at discovering new treatments for myelodysplastic syndromes (MDS) and leukemias, and his research has shown that the TGF-beta superfamily/smad, IRAK and MAP kinase pathways are overactivated in MDS. His work has led to clinical trials with Luspatercept, Galunisertib, CA4948, ARRY-614 and various other promising agents in MDS. DR. Verma&rsquo;s research has shown that MDS and leukemias are caused by aberrant leukemic stem cells and has identified novel targets against MDS/AML stem cells that are being tested in various studies.</p><p>Dr. Verma is a scholar of the Leukemia and Lymphoma society, elected member of American society of clinical investigation and his research is funded by the NIH, Department of Defense, American Cancer Society, Edward P. Evans Foundation and various other private foundations.</p>

<p>Roberto Alejandro Sica, MD, is an Attending Physician at Montefiore and an Assistant Professor of Medicine at our Albert Einstein College of Medicine. Since joining the Montefiore team, Dr. Sica&rsquo;s clinical focus has been on chimeric antigen receptor (CAR) T-cell therapies, immunotherapies and allogeneic stem cell transplantation, central nervous system (CNS) lymphomas and HIV, ATLL and gene therapy for malignant and nonmalignant hematologic disorders. Dr. Sica is a member of the Bone Marrow Transplant team at Montefiore.</p><p>Dr. Sica attended the University of Buenos Aires School of Medicine, where he received his Medical Degree. During his time there, he also served as an instructor of immunology, virology, pharmacology, and physical diagnosis. Dr. Sica began his postgraduate training with a residency in Internal Medicine at the Carney Hospital/Tufts University School of Medicine in Boston. Following this, he completed a fellowship in Hematology/Oncology at the University of Illinois at Chicago, where he received the E. Donnall Thomas Achievement in Medicine Award for his academic performance and the Edward Jenner Innovation in Medicine Award in recognition of his innovative contributions to the design of clinical protocols and trials. Later, he completed a fellowship in Blood and Marrow Transplant and Cellular Therapies at Stanford University. At that institution, he performed research with Dr. Sally Arai on Hodgkin's lymphoma using the Stanford-pioneered GN-BVC regimen (Gemcitabine, Vinorelbine-BCNU. Etoposide and Cyclophosphamide) which resulted in decreased toxicity and improved overall survival in patients. His work has been published in various review journals and abstracts.</p><p>Dr. Sica&rsquo;s research focuses on novel cellular therapies, allogeneic bone marrow transplantation for malignant and nonmalignant hematologic disorders, and combination immunotherapy and cancer vaccine.</p><p>Dr. Sica is board certified in Internal Medicine, Hematology and Medical Oncology by the American Board of Internal Medicine.</p>

<p>I am a board certified hematologist and medical oncologist. I specialize in caring for patients with hematologic malignancies and specifically myelodysplastic syndromes (MDS), acute myeloid leukemia (AML) and aplastic anemia (AA).</p>
<p>I have a strong interest in exploring novel treatment approaches for myelodysplatic syndromes (MDS) and Acute Myeloid Leukemia (AML) after the failure of therapy with hypomethylating agents. I specifically focus on therapeutically targetting the transcription factor STAT3 in MDS and AML.</p>
<p>My research was previously funded through the NIH Paul Calabresi Career Development Award (K12) &amp; currently through the Leukemia Lymphoma Society (LLS)-TRP Award &amp; MDS Foundation Gilbert Bruce Smith Young Investigator Award.</p>
<p>My original research and reviews have been published in leading peer reviewed journals that include Blood, JCI, Nature Reviews Cancer &amp; Lancet Hematology.&nbsp; I have been an invited speaker at several conferences and forums nationally as well as internationally.</p>

He went on to complete a residency in medicine and fellowship in hematology/medical oncology at Duke University Medical Center. Since then Dr. Packer has held clinical and teaching positions at leading cancer centers, including Memorial Sloan-Kettering Cancer Center, Mount Sinai School of Medicine and Montefiore Einstein Center for Cancer Care.</p>
<p>Dr. Packer&rsquo;s clinical expertise is in medical management of lung cancer, head and neck cancer and prostate cancer. He is director of the melanoma and sarcoma programs at Montefiore Einstein Center for Cancer Care. In addition, Dr. Packer is medical director of Montefiore&rsquo;s Oncology Care Model (OCM), an alternative payment model sponsored by the Centers for Medicare and Medicaid Services and aimed at improving patient care coordination and appropriateness of care.</p>

<p><strong>The Roles of Signaling Pathways in Adult Blood Development and Leukemia</strong></p>
<p><span style="font-size: 10.5pt; font-family: Verdana, sans-serif;">The Gritsman lab studies the signal transduction pathways that affect the early fate decisions of adult hematopoietic stem cells (HSCs) as they progress from an undifferentiated multipotent state to the generation of differentiated blood cells.&nbsp; When these early fate decisions go awry, this can lead to the formation of leukemia-initiating cells. We are interested in</span><span style="font-size: 10.5pt; font-family: Verdana, sans-serif;">&nbsp;how signaling pathways affect the self-renewal and differentiation of HSCs and malignant or pre-malignant stem cells in myeloid malignancies, such as acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), and myeloproliferative neoplasms (MPN).</span></p>
<p><strong>Roles of the PI3 kinase isoforms in adult blood development</strong></p>
<p>PI3 kinase (PI3K) is a lipid kinase that is important for the regulation of metabolism, the cell cycle, apoptosis, and protein synthesis. &nbsp;In hematopoietic cells, there are four isoforms of the catalytic subunit of PI3K, each encoded by a separate gene. &nbsp;Emerging evidence suggests that these isoforms have unique functions in normal and cancer cells, but may substitute for each other in some contexts.&nbsp; We have generated a series of mouse knockout models that allow us to study the roles of each of these isoforms individually in adult hematopoiesis.&nbsp; For example, we have found that the p110alpha isoform is most important for red cell development, but is not required in normal blood stem cells. We have now also generated compound knockout mice to determine the redundant roles of the PI3K isoforms in blood development. &nbsp;We recently reported that PI3K isoforms play important redundant roles during the hematopoietic stress response, such as after chemotherapy. However, deletion of all 3 Class IA PI3K isoforms leads to a phenotype with impaired HSC differentiation, resembling myelodysplastic syndrome (MDS). We are studying how deletion of PI3K will impact normal HSC function, including self-renewal, proliferation, and differentiation along different blood lineages by affecting processes such as autophagy and epigenetic regulation in HSCs.</p>
<p><strong>Roles of the PI3 kinase isoforms in leukemia</strong></p>
<p>Acute myeloid leukemia (AML) is a genetically diverse disease, but activation of the PI3K pathway has been reported in up to 80% of cases.&nbsp; A subset of AML cell lines and AML patient samples respond to PI3K pathway inhibitors, but it is unclear how patients should be selected for potential response to these inhibitors. &nbsp;We found that RAS-mutated myeloid leukemias are particularly dependent on the p110alpha isoform of PI3K, and that pharmacologic inhibition of p110alpha can be used to treat both RAS-mutated cell lines and RAS-mutated leukemia in mice. Furthermore, we use cell lines, patient samples, and mouse models of leukemia to investigate the mechanisms of resistance to PI3K inhibition, with the goal of identifying new drug targets and designing new combination treatments for leukemia that incorporate PI3K inhibitors.</p>
<p><strong>RON Kinase in Myeloproliferative Neoplasms</strong></p>
<p>The myeloproliferative neoplasms (MPNs) are a group of diseases that are caused by kinase mutations in HSCs, which lead to uncontrolled proliferation of myeloid cells. The Philadelphia chromosome-negative MPNs are characterized by mutations in the JAK/STAT signaling pathway, and respond to JAK inhibitors, but resistance often develops. We recently discovered that the receptor Tyrosine kinase RON can physically interact with JAK2 in MPN cells, leading to potentiation of JAK/STAT signaling in resistant cells. Furthermore, we found that pharmacologic or genetic inactivation of RON can inhibit proliferation of MPN cells and re-sensitize resistant cells to JAK inhibitors.</p>

<p class="MsoNormal" style="margin: 0in 0in 0.0001pt; font-size: medium; font-family: 'Times New Roman', serif;"><span style="font-size: 10.5pt; font-family: Verdana, sans-serif;">&nbsp;</span><span style="font-size: 10.5pt; font-family: Verdana, sans-serif;">The Gritsman lab&rsquo;s research interests include the contributions of signaling pathways to leukemic and pre-leukemic stem cell dormancy in minimal residual disease, which includes mechanisms of immune evasion. Furthermore, the Gritsman lab is interested in the roles of inflammatory signaling pathways and of the local microenvironment in bone marrow fibrosis, and in the evolution of myeloid neoplasms from the pre-malignant to malignant state. Our major goals are to identify opportunities for therapeutic targeting to prevent the transition from the pre-leukemic state to leukemia, or to eliminate minimal residual disease to prevent relapse.</span></p>

<p>Dr. Geller is a full-time faculty member of the Orthopaedic Surgery Department at Montefiore Medical Center and serves as the director of the Orthopaedic Oncology Service, managing both pediatric and adult patients.&nbsp; He is the co-director of Orthopaedic Surgery at the Montefiore Medical Center Moses Campus and currently serves as the Quality Improvement Officer for the Montefiore Cancer Committee.&nbsp; His clinical focus involves primary and metastatic bone and soft tissue tumors, with a secondary interest in joint replacement in the setting of extensive bone loss.&nbsp; He has experience in navigation-guided surgery, allograft reconstruction, endoprosthetic reconstruction, and use of custom or patient-tailored reconstructive prostheses.&nbsp; His research interests include expression patterns in osteosarcoma, surgical margins in osteosarcoma, novel drug screening methods and targeted therapy for osteosarcoma using novel therapeutic modalities.&nbsp; He is extensively involved in resident and medical student education.</p>

Biography

<p>David S. Geller is a board-certified orthopedic oncologist who specializes in the treatment of both pediatric and adult patients with bone and soft-tissue tumors. He has been a full-time faculty member at Montefiore since 2006. He is an Associate Professor of Orthopedic Surgery and Pediatrics at our Albert Einstein College of Medicine.</p><p>Dr. Geller is known for his expertise in musculoskeletal tumor surgery, with specific experience in bone and soft-tissue sarcomas, such as osteosarcoma, Ewing's sarcoma, liposarcoma and synovial sarcoma. He has extensive experience in managing patients with metastatic carcinoma of the bone as well. He routinely offers limb-salvage surgery using a wide array of reconstructive tools.</p><p>Dr. Geller earned his medical degree from Tel Aviv University's Sackler School of Medicine and completed an orthopedic surgery residency at Montefiore. He received his fellowship training from the Harvard Combined Orthopedic Oncology Program, which included Massachusetts General Hospital, Boston Children's Hospital, and Beth Israel Deaconess Medical Center.</p><p>Dr. Geller serves as the Associate Director of the Musculoskeletal Oncology Research Laboratory at Montefiore. He leads numerous translational research efforts designed to investigate the pathophysiology and molecular mechanisms of bone and soft tissue cancers. He is interested in improved surgical techniques and novel therapeutic approaches for primary malignant bone tumors. He is actively involved in collaborative clinical trials both nationally and internationally.</p><p>Dr. Geller is an active member of the American Academy of Orthopedic Surgeons, Muskuloskeletal Tumor Society, Connective Tissue Oncology Society, Children's Oncology Group, American Orthopedic Association, and the American Association for Cancer Research.</p>
