<p>Jeffrey Weiss, MD is a senior clinician-executive who has taken on progressive leadership positions at Montefiore Health System. Dr. Weiss earned a bachelor of science in industrial and labor relations from Cornell University, a medical degree from Stony Brook School of Medicine, and the Internal Medicine Residency Program at Montefiore, where he was chief resident.</p>
<p>In 2007, Dr. Weiss was named Institutional Medical Director for Montefiore Health System. His role is to help optimize the performance of the Montefiore health delivery network by ensuring a smooth interface between its various components - the doctors, hospital, medical group, home health agency, nursing homes, and the CMO. Dr. Weiss's responsibilities included leadership of nationally recognized patient safety, quality and peer review programs, management of a medical staff with 3,500 employed and voluntary physicians, supervision of GME activities for 1,200 residents in the second largest house staff program in the country, all under the coordinated umbrella of the "Office of the Medical Director" (OMD). As part of a restructuring of organizational clinical leadership in 2012, Dr. Weiss was named Vice President for Medical Affairs. In this role, he has retained many of his previous responsibilities, and is now leading a team of five newly appointed hospital medical directors in driving physician alignment and creating a horizontally integrated approach to quality and clinical performance across the system.</p>
<p>Dr. Weiss is an active clinician, teacher and researcher. He lectures nationally on topics including leadership, physician alignment, and patient safety in several forums, including as a faculty member for the AAMC and National Patient Safety Foundation.</p>

<p class="MsoNormal">Karen Warman, MD, MSc, provides general pediatric care for children from infancy through adolescence and teaches medical students and residents.</p><p class="MsoNormal">Dr. Warman is the founder and director of the Pediatric Preventive Asthma Care Program at Montefiore&rsquo;s Comprehensive Family Care Center. Regionally, she serves on the New York City Asthma Partnership (NYCAP) Steering Committee and co-chairs NYCAP&rsquo;s Environment Group, which focuses on attaining habitable housing for children with asthma to reduce asthma morbidity. She also serves as a consultant for the American Lung Association-Northeast and the NYC Department of Health. In the latter capacity, she trains school nurses in New York City to manage asthma in the schools.</p><p class="MsoNormal">Dr. Warman has taken a leadership role in several asthma programs. These include: serving as site leader for a Center for Disease Control and Prevention (CDC) sponsored, multi-state replication of the National Cooperative Inner-City Asthma Study (NCICAS); conducting the Physician Asthma Care Education (PACE) training program; and creating, implementing and evaluating the Asthma Passport Program.</p><p class="MsoNormal">Dr. Warman graduated from SUNY Upstate Medical School and completed her residency at Columbia-Presbyterian Medical Center, which is now part of New York-Presbyterian/Columbia University Medical Center.</p>

She is Program Director of the Addiction Medicine Fellowship and Co-Director of the Addiction Medicine rotation for medical trainees. An addiction medicine and internal medicine physician, she leads the development and implementation of health system interventions to improve substance use disorder (SUD) care in acute care settings and transitions of care to post-acute and ambulatory care settings.</p>
<p class="MsoNormal" style="margin: 0in; font-size: medium; font-family: Calibri, sans-serif;"><br />Dr. Torres-Lockhart provides SUD treatment, HIV/AIDS care, hepatitis C treatment, and general internal medicine care at several sites across Montefiore Medical Center, including a community health care center. She also actively teaches and supervises medical students, residents, and fellows. She is double board certified in addiction medicine and internal medicine and credentialed as an HIV specialist by the American Academy of HIV Medicine.&nbsp;<br /><br />Dr. Torres-Lockhart completed her fellowship training in addiction medicine at Montefiore Medical Center. Prior to arriving at Montefiore, she completed her residency training in internal medicine and primary care at Brigham and Women&rsquo;s Hospital in Boston, MA, where she also served as primary care chief medicine resident at the West Roxbury and Jamaica Plain Veterans Affairs Medical Centers. She received her undergraduate degree from Dartmouth College and her medical degree from the Geisel School of Medicine at Dartmouth. She is the PI for a HRSA training grant and co-PI for an OASAS training grant to lead the development of educational opportunities in building the addiction medicine workforce. She currently serves on the Board of Directors for the New York Society of Addiction Medicine previously as Communication Chair and now as Secretary.&nbsp;</p>

<p>Kristine E. Torres-Lockhart, MD, FASAM, is Director, Addiction Medicine Fellowship Program at Montefiore Einstein, Founding Director, Addiction Consult Service at Weiler Hospital and Assistant Professor, General Internal Medicine and Psychiatry and Behavioral Sciences at our Albert Einstein College of Medicine. Dr. Torres-Lockhart is a general internist, primary care provider and addiction medicine specialist. She focuses on caring for people who use substances and people with substance use disorders, providing care in hospital settings and in outpatient clinics.</p><p>After obtaining her Bachelor of Arts in neuroscience from Dartmouth College in 2010, Dr. Torres-Lockhart earned her Doctor of Medicine at the Geisel School of Medicine at Dartmouth in 2016. She completed an internship and residency in internal medicine and primary care at Brigham and Women&rsquo;s Hospital in 2019. Following this, Dr. Torres-Lockhart completed a fellowship in addiction medicine at Montefiore Einstein in 2020.</p><p>Dr. Torres-Lockhart's research focuses on evidence-based and harm-reduction-oriented strategies for addressing substance use disorder care in acute care settings, integration of addiction medicine into medical education and optimizing transitions of care for people who use drugs. She has been principal investigator and co-investigator on funded research projects and her work has been published in numerous peer-reviewed publications. Dr. Torres-Lockhart has also shared her work through numerous invited presentations, abstracts and poster presentations. She is a reviewer for scientific journals, including Addiction Science and Clinical Practice, the Journal of Addiction Medicine, the Journal of Substance Abuse Treatment and the Journal of Hospital Medicine.</p><p>Dr. Torres-Lockhart is board certified by the American Board of Internal Medicine and the American Board of Preventive Medicine with certification in Addiction Medicine. She is a Fellow of the American Society of Addiction Medicine, board member of the New York Society of Addiction Medicine and member of the Association for Multidisciplinary Education and Research in Substance Use and Addiction. In 2022, Dr. Torres-Lockhart received the President&rsquo;s Award at the New York Society of Addiction Medicine&rsquo;s Annual Conference. In 2023, she received the Quality Improvement Champion honor at the United Hospital Fund's Tribute to Excellence in Health Care and was a Rising Star Nominee for the Department of Medicine&rsquo;s Physician Recognition Awards at Montefiore Einstein.</p>

<p>Andrew Telzak, MD, MSc, is an attending physician and Director, New York City Research and Improvement Networking Group (NYC RING) at Montefiore Einstein and Assistant Professor, Family and Social Medicine and Pediatrics at our Albert Einstein College of Medicine. Dr. Telzak&rsquo;s clinical focus is on providing comprehensive, high-quality primary care to families and communities. He has expertise in providing complex care coordination for patients with both medical and social needs.</p><p>After receiving his Bachelor of Arts in public health studies from The Johns Hopkins University in 2008, Dr. Telzak earned his Doctor of Medicine at Montefiore Einstein in 2016. He remained at this institution to complete his family medicine residency in 2019, where he was a Chief Resident in his final year. Following this, Dr. Telzak earned his Master of Science in clinical research methods at Montefiore Einstein in 2021 and completed a population health fellowship the same year.</p><p>Dr. Telzak&rsquo;s research focuses on the intersection of social determinants of health and clinical outcomes for patients with chronic illnesses. He has been principal investigator and co-investigator on several grant-supported research projects and has shared his work through numerous peer-reviewed journals, abstracts, posters, book chapters and invited presentations. Dr. Telzak is also a reviewer for scientific journals, including <em>Circulation</em>, the <em>Journal of the American Heart Association</em>, <em>BMC Primary Care</em> and the <em>American Journal of Preventive Medicine</em>.</p><p>Dr. Telzak is board certified in Family Medicine. He is a member of the American Academy of Family Physicians and the North American Primary Care Research Group (NAPCRG).</p>
