Alumni and former students (Feb 2019 and prior) with requests for transcripts, verification letters, and replacement diplomas, please follow the instructions below:

To obtain your official graduate transcript, please follow the instructions provided on the Yeshiva University website: www.yu.edu/transcript  

If you need verification of your PhD or MS degree, please submit a request through the National Student Clearinghouse at www.degreeverify.org , the authorized agent for providing PhD and MS degree verifications.

Please use school code 002903 – Yeshiva University - Albert Einstein College of Medicine.

To request a duplicate diploma, please complete and submit the Request for Duplicate Diploma Form, available here.
