Professional Interests

<p><strong>Betsy Herold, M.D.</strong> directs a basic and translational research program, which focuses on virus host interactions. Projects in the lab include studies designed to identify the cellular signaling pathways that herpes simplex viruses (HSV) usurp to promote viral entry and infection. The lab uncovered a previously unappreciated paradigm associated with activation of phospholipid scramblases, which are known to catalyze the movement of phosphatidylserine lipids between the inner and outer leaflet of the plasma membrane. Surprisingly, they found that the exofacial movement of phospholipids is associated with concomitant translocation of intracellular proteins, including the master kinase Akt to the outside, where Akt becomes phosphorylated to activate an &ldquo;outside-inside&rdquo; signaling cascade that promotes viral entry. This pathway is also usurped by SARS-CoV-2 and is important for cellular processes including apoptosis. In collaboration with the Almo lab, they have engineered cell impermeable kinase inhibitors. These compounds block viral entry and prevent induction of apoptosis by select TNF ligands.</p>
<p> Serendipitously, in studying this signaling pathway, the lab identified a novel candidate vaccine for the prevention and treatment of HSV infections.  Most efforts to develop a vaccine have focused on neutralizing antibodies that target HSV glycoprotein D (gD), but all of these have failed in clinical trials. Instead, the lab (in collaboration with the Jacobs lab), engineered a virus completely deleted in gD. Glycoprotein D is required for viral entry and cell-to-cell spread, thus the deletion virus (DgD-2) is restricted a single cycle and will not spread. This candidate vaccine elicits T cell responses and high titer, polyfunctional antibodies that protect through antibody-dependent cell mediated cytotoxicity (ADCC). The vaccine prevents the establishment of latency in mice and is significantly more protective in multiple small animal models than prior vaccines that have failed in clinical trials. The lab has subsequently isolated monoclonal antibodies (mAbs) that have this protective ADCC activity and both the vaccine and the mAbs are being advanced for preclinical development.  Studies to understand why this vaccine elicits ADCC-mediating antibodies whereas gD vaccines and primary HSV infection only elicit neutralizing antibodies led to the identification of a key role for TNFRSF14 (aka HVEM) in generating and mediating ADCC responses. HVEM is an immune cell surface protein that functions in signal transduction pathways that regulate inflammatory or inhibitory immune responses but its role in shaping the B cell repertoire and in providing a second signal for ADCC had not been previously described and has implications for vaccine development and oncolytic therapies. </p>
<p> The third major area of basic research involves defining the molecular mechanisms underlying the HIV-HSV syndemic. Epidemiological studies have consistently demonstrated that being HSV seropositive is associated with an increased risk for HIV acquisition, replication, higher plasma viral loads and more frequent episodes of HIV reactivation.  Using primary cells from patients and HIV latently infected cell lines, the lab has identified several mechanisms by which HSV promotes HIV latency reversal and replication including upregulation of the noncoding RNA, <em>Malat1</em>, and downregulation of IL-32. Defining these pathways may lead to identification of new strategies to &ldquo;shock and kill&rdquo; or &ldquo;block and lock&rdquo; HIV.</p>
<p> Clinical studies include prevention of infectious disease complications in transplantation. Members of the research group are involved in studies to optimize pre-emptive prophylaxis for CMV and EBV, vaccine responses in transplantation recipients, and others</p><p>Studies with vaginal microbicides have resulted in the expnasion of studies to focus on soluble mucosal immunity in the genital tract. We found that .female genital tract secretions collected from healthy women provide variable, but significant protection against both HSV and HIV. Mechanistic studies suggest that this endogenous activity is mediated by defensins and other antimicrobial peptides. This endogenous activity may serve as a biomarker of a "healthy mucosal immune environment" and thus provide a surrogate marker to evaluate the safety of vaginal microbicides. In addition, identification of the mediators that contribute to this endogenous activity could lead to development of new strategies to boost this host defense and help protect against infection. These studies are being conducted in collaboration with the proteomics core facility at AECOM. Additionally, we are testing the hypothesis that HSV triggers changes in the mucosal environment, which allow it to escape cervical secretion defenses, enhance its own infectivity and facilitate HIV co-infection. Our preliminary observations support the paradigm that HSV disrupts the epithelial barrier by targeting tight junction and adherens junction proteins, and interferes with host defenses by triggering an inflammatory response and a loss in protective proteins such as SLPI. These changes could facilitate both its own infectivity and enhance HIV co-infection.<br /><br /></p>
<p>Results obtained from this bench research are critical to the laboratory's translational studies. The focus of the Translational Microbicide Research Program is to identify optimal combinations of topical microbicides that are safe and target different steps in HIV life cycle, thus reducing the risks of drug resistance and providing greater protection than could be achieved with a single agent, and also target HSV infection. Candidate combinations are evaluated using a multi-tiered approach for anti-viral activity and safety using human cervical cultures, as well as primary T cells and macrophages, in the presence of cervicovaginal secretions and seminal plasma. Leading combinations are then evaluated in human explant cultures (cervical, vaginal) and in murine genital models and a new cotton rat model for anti-viral activity and for the impact on mucosal immunity. If results of these pre-clinical studies suggest that candidate microbicides are safe and effective, the drugs are advanced for regulatory testing, and undergo evaluation in Phase I clinical studies.<br /><br /></p>

<p>Betsy Herold, MD, is Chief of the Division of Pediatric Infectious Diseases and Vice Chair for Research at Children's Hospital at Montefiore. Dr. Herold is also a Professor of Pediatrics, Microbiology &amp; Immunology, and Obstetrics &amp; Gynecology and Women's Health at our Albert Einstein College of Medicine. She specializes in pediatric infectious diseases. </p><p>Dr. Herold received her Bachelor of Arts in Biology in 1978 from Brown University and went on to receive her Doctor of Medicine from the University of Pennsylvania Medical School in 1982. Dr. Herold completed an internship and residency in pediatrics at Children's Memorial Hospital, where she became a Chief Resident in 1985. She then began a fellowship in research at Hagedorn Research Laboratory in Gentofte, Denmark. <p>Samuel Gorstein, MD, is Director, Resident Education, Pediatric Intensive Care Unit (PICU) at the Children&rsquo;s Hospital at Montefiore Einstein (CHAM) and Assistant Professor, Pediatrics at our Albert Einstein College of Medicine. Dr. Gorstein&rsquo;s clinical focus is on the care of critically ill infants, children and adolescents, focusing on conditions requiring intensive monitoring, life support and complex interventions, such as respiratory failure, sepsis, trauma and post-surgical recovery.

<p>Dr. Fuloria, Associate Program Director for the Neonatal-Perinatal Medicine Fellowship Program at The Children&rsquo;s Hospital at Montefiore, obtained her medical degree from King George&rsquo;s Medical College in Lucknow, India, and then did her pediatric residency and neonatal-perinatal medicine fellowship at Wake Forest University School of Medicine in Winston-Salem, NC. <p>Alexandre N. Darani, MD MSc, is Pediatric Surgeon at Montefiore and Assistant Professor of Surgery at our Albert Einstein College of Medicine. His clinical focus is surgery in children and newborns. He also specializes in pediatric hepatobiliary surgery, including the management of portal hypertension, liver and kidney transplant surgery, minimally invasive thoracic and abdominal surgery, neonatal surgery, oncology and short bowel syndrome management.<br /><br />In 1999, Dr. Darani received his Doctor of Medicine from the Federal Public Health Office of Switzerland after completing Medical School at the Faculty of Medicine at the University of Lausanne. In 2001, Dr. Darani completed a Master of Science in Biomedical Engineering at the Polytechnic School of Montreal and the Research Centre at St. Justine Children&rsquo;s Hospital in Canada, focusing on biomaterials and tissue engineering. He returned to Switzerland to continue his training, completing residency in Pediatric Surgery at Lausanne University Hospital in 2002, General Surgery residency at the Jura-Bernois Canton Hospital in 2004, and Neonatology Intensive Care at Lausanne University Hospital in 2005. To follow, Dr. Darani completed 3 years of specialization in Pediatric Surgery at the Red Cross Children&rsquo;s Hospital through the University of Cape Town before returning to Geneva. He became Fellow in Pediatric Surgery and Urology at the University Children&rsquo;s Hospital at the University of Geneva in 2011. After working one year as an Attending Pediatric Surgeon in the Italian side of Switzerland, he subspecialized in pediatric hepatobiliary and transplant surgery completing a three-year Pediatric Transplant Fellowship at the Children&rsquo;s Hospital at Westmead in Sydney, Australia in 2016 and a two-year Fellowship in Transplant Surgery at the Northwestern Memorial Hospital and Lurie Children&rsquo;s Hospital in Chicago in 2018.<br /><br />Dr. Darani&rsquo;s clinical research work has been published in numerous peer-reviewed journals and chapters and has been presented nationally and internationally. His current research focus on translational research related to biomedical engineering like hepatic and enteric regenerative medicine and new development in minimal invasive surgery. <br /><br />In 2015, Dr. Darani received the Lorimer Teaching Award for clinical surgical teaching, given to him by the Sydney Medical Program at the University of Sydney. He is board certified by the FMH College of Medicine of Switzerland and a member of numerous professional societies, including the American Society of Transplant Surgeons, the Swiss Pediatric Surgeons Society and the International Pediatric Transplantation Association.</p>

<p>Pe&rsquo;er Dar, MD, is Director of Fetal Medicine and OBGYN Ultrasound at Montefiore and Professor of Obstetrics and Gynecology and Women&rsquo;s Health at our Albert Einstein College of Medicine. Dr. Dar&rsquo;s clinical focus is on the care of women who carry babies with disorders and medical conditions that may affect their lives and he leads the Fetal Medicine and Surgery Team at Montefiore. He particularly specializes in diagnosis and management of medical and structural abnormalities in the fetus and in complicated twin and triplet pregnancies. His expertise include performing detailed ultrasound examination of the fetus including fetal echocardiography (scanning of the fetal heart) and fetal neurosonography (scanning of the fetal brain) as well as performing in-utero fetal surgeries such as fetoscopic laser surgery for monochorionic twins with twin to twin transfusion syndrome, in-utero blood transfusion for fetuses with anemia and in-utero drainage and shunt procedures. Dr. Dar is also experienced in performing amniocentesis, chorionic villous sampling (CVS) and multifetal selective reduction for multiple pregnancies.&nbsp;&nbsp;&nbsp;&nbsp;</p><p>Dr. Dar completed his Doctor of Medicine from Tel-Aviv University in Tel-Aviv, Israel in 1986. He started his postgraduate education at Assaf Harofe Medical Center, Tel-Aviv University, with a residency in obstetrics and gynecology and a specialty in high-risk pregnancies from 1992 through 1998. He then came to Montefiore-Einstein, completing a two-year fellowship in medical and reproductive genetics. After spending two years as an attending-specialist in the field of prenatal diagnosis he completed in 2005 three-year residency in obstetrics and gynecology at Montefiore-Einstein. In 2007, he completed additional training in fetal surgery in world-known fetal surgery centers in France and Belgium. &nbsp;</p><p>Dr. Dar is very active in research and he is currently the principal investigator in many studies including international multicenter studies. His research focus is in fetal medicine and surgery, non-invasive prenatal screening and placenta accreta spectrum. He has shared his work in many peer-reviewed journals, textbook chapters, abstracts and invited presentations. He is also a reviewer for national and international journals including the <em>American Journal of Obstetrics and Gynecology, Plos one and Prenatal Diagnosis. </em></p><p>Dr. Dar is board certified in both Obstetrics and Gynecology and Clinical Genetics and is a member of several professional societies including the Society of Maternal and Fetal Medicine, International Fetal Medicine and Surgery Society and the International Society of Ultrasound in Obstetrics and Gynecology. She conducted the Low-birth-weight infant Evaluation and Assessment Program (LEAP) clinic from 1988 through December 2022 and served as the Chief of Neonatology&nbsp;from 1994 until 2023. Her research and clinical interests include health services delivery, global perinatal heath care, perinatal ethics, shared decision-making and palliative care education, family integrated care in the neonatal intensive care unit and&nbsp;neonatal outcomes for very preterm and medically complex infants.&nbsp;</span><span style="font-size: 10.0pt; color: black; border: none windowtext 1.0pt; mso-border-alt: none windowtext 0in; padding: 0in;">&nbsp;</span></p>

<p>Dr. Campbell, Program Director for the fellowship in Neonatal-Perinatal Medicine and Director of the Division of Neonatology at The Children&rsquo;s Hospital at Montefiore, conducts the Low Birth Weight Infant Evaluation and Assessment Program (LEAP). She focuses on perinatal care and health services delivery, psychosocial support interventions for mothers of premature babies, perinatal ethics and collaborative decision-making, outcomes for very low birth weight infants and newborn hearing screening. She is a co-chair of the Faculty Mentoring Work Group of the Education and Faculty Support Committee at Einstein and led a faculty development initiative within the Department of Pediatrics.</p><p>A member of the National March of Dimes Bioethics Committee and NICU Family Support Project Advisory Group, Dr. Campbell also serves on the New York City and Bronx Infant Mortality Review Committees and is a co-chair of the Bronx Regional Perinatal Forum. She is a member of the Greater New York Hospital Association Perinatal Safety Collaborative Advisory Group and the National Quality Forum Perinatal Collaborative and the New York State Obstetric and Neonatal Quality Collaborative, Neonatology Expert Workgroup. Dr. Campbell also serves on the New York State Palliative Care Education and Training Council. Dr. Campbell is the recipient of the 2000 Bronx Perinatal Consortium Distinguished Clinical Care Award, the 2005 New York State Perinatal Association L. Stanley James Award and the 2009 National Perinatal Association Stan Graven Award. She has been recognized by Castle Connolly and the New York Magazine in their lists of Best Doctors.</p>

Biography

<p></p>Steven Howard Borenstein, MD, is a Pediatric Surgeon at Children's Hospital at Montefiore and an Assistant Professor of Surgery at our Albert Einstein College of Medicine. Since joining the Montefiore team in 2006, Dr. Borenstein's clinical focus has been surgical oncology, minimally invasive surgery, and neonatal surgery.<br /><br />In 1988, Dr. Borenstein received his Bachelor of Science at McGill University. He then attended Dalhousie University, where he received his Doctor of Medicine in 1992. Dr. Borenstein began his postgraduate training in 1992 with a year-long rotating internship at Dalhousie University followed by a residency in General Surgery at the University of Toronto. During his residency, Dr. Borenstein was engaged in research on neonatal models of transplantation tolerance and received his Doctor of Philosophy with the University of Toronto's Institute of Medical Science in 1999. Dr. Borenstein continued his clinical training with the University of Toronto and completed a fellowship in Pediatric General Surgery at The Hospital for Sick Children in 2003.<br /><br />In 2003, Dr. Borenstein became a fellow of the Royal College of Physicians and Surgeons of Canada with specialty training in pediatric general surgery. He is also a member of numerous professional societies, including the Canadian Association of General Surgeons and the Canadian Association of Pediatric Surgeons.<br /><br />In relation to his clinical focus, Dr. Borenstein has been a co-author on many peer-reviewed articles dealing with various problems in pediatric surgery such as: malrotation, Hirschsprung's disease and intussusception.<br /><br />Dr. Borenstein was an attending pediatric surgeon at McMaster University in Hamilton, Ontario from 2003-2006 before moving to New York with his family and joining the Department of Surgery at Montefiore.<p></p>
