<p style="font-family: Arial; font-size: 11.0pt;"><span style="font-family: 'Times New Roman'; font-size: medium;">Jessica Zwerling, MD, MS, has a clinical focus on identifying risk factors for cognitive decline and dementia, as well as factors that promote successful aging. Zwerling’s research focuses on optimizing healthcare delivery systems. Her work has been published in many peer-reviewed journals and has been shared at national and international invited conferences. She has developed the clinical and didactic programs used to enhance health professionals’ capacity to screen, diagnose, and develop personalized plans of care for patients with Alzheimer disease and related dementias for the Montefiore Health System. She also is a nationally recognized expert in aging/ dementia and serves as an adviser on multiple panels related to recruitment in neurodegenerative disease clinical trials. In addition, she has pioneered the age-friendly initiatives at her health system and certified her Center of Excellence with the highest age-friendly recognition. Zwerling is board certified in neurology as well as the subspecialty of neuromuscular medicine by the American Board of Psychiatry and Neurology. She is a member of the American Geriatrics Society and the American Academy of Neurology. Additionally, she has been named “2020 Health Care Hero” by the Westchester magazine, and was named one of the New York Super Doctors in The New York Times from 2016 to 2023. She has also been appointed and completed the Physician Leaders Program at Montefiore Health System.</span></p>

Clinical Focus

<p>To meet the needs clinically of our patients/caregivers, I developed and published a new model of care for the community. This model is called the Coordinated Care At Risk/Remote Elderly Program (CCARRE) and focuses on the patient/caregiver dyad. Additional funding supported the expansion of a telehealth depression treatment program for older adults which supported recruitment in our diverse, frail population. Foundation support has provided the ability to provide culturally and language competent assessments/evaluations and management of neurodegenerative diseases and cognitive impairment. Other grant funding supports the implementation of the 5cs program (Comprehensive Culturally relevant Care for Cognitively impaired and their Caregivers – a pathway to equity in health care and model for systems transformation. Current funding for BRAID in conjunction will bridge our research aims and This grant uses the MHS-pioneered BRAID model (Bridging Research, Accurate Information, and Dialogue) where researchers/providers gain a deep understanding of the needs of our diverse and vulnerable population. To aid in clinical trial enrollment/retention, this work focuses on the Age Friendly 4Ms--What Matters, Medication, Mentation, and Mobility and strengthens the collaboration between the health system and the surrounding community.
</p></p>Educational models through research: Additional national funding supported the building of a network of skilled nursing facilities and expanded my role as a leader of a Covid Action Network through Project ECHO. I developed the curriculum for interprofessional and brought the community of 99 skilled nursing facilities together with our Montefiore Health System experts to provide evidence-based Covid 19 care. In addition - I obtained funding to implement the MOLST or eMOLST into our collaborative of skilled nursing facilities to assure safe care transitions and encourage the end-of-life directives to be completed electronically.</p>

Research Focus

My overall clinical research focuses on how to address the neurodegenerative disease. The pandemic has forced a reevaluation of resources and additional funding to support a robust educational network that provides interdisciplinary CME, as well as telehealth for the older adult, which was crucial. Building a patient-caregiver evaluation center through foundation and state/national funding is crucial and has created a niche as an expert in neurodegenerative disease with a focus on under resourced, frail populations. This group is crucial to identify and recruit for future trials and our site has been identified as a patient-centered outcome center.

Her clinical focus is on identifying risk factors for cognitive decline and dementia, as well as factors that promote successful aging.</p><p>In 1996, Dr. Zwerling received her Bachelor of Science in human development and family studies at Cornell University, as well as earned a concentration in Gerontology. She continued her education at Roswell Park Cancer Institute/University at Buffalo, focusing on the validation of dementia rating scales, receiving her Master of Science in 2000 and her Doctor of Medicine in 2001, She earned her Doctor of Medicine with both thesis and research honors. Her postgraduate training began with an internship at Beth Israel Medical Center, followed by a residency/chief residency at Montefiore Medical Center. In 2006, she completed a fellowship in neuromuscular disease, electromyography and neurophysiology at Montefiore. <p>Jacqueline Weingarten-Arams, MD, is an attending physician and Professor, Pediatrics at Montefiore Einstein. Dr. Weingarten-Arams&rsquo; pediatric critical care expertise focuses on cardiac critical care, acute hypoxemic respiratory failure, nutrition in critical illness and energy expenditure, cardiopulmonary resuscitation (quality CPR) and extracorporeal life support (ECMO).</p><p>After obtaining her Bachelor of Science in nutritional biochemistry at Cornell University in 1982, Dr. Weingarten-Arams earned her Doctor of Medicine at the same institution in 1986. She then completed her pediatric residency at Columbia University in 1990, where she was Chief Resident in her final year. Following this, Dr. Weingarten-Arams completed a fellowship in pediatric critical care medicine at New York Hospital Cornell University Medical College in 1996.</p><p>Dr. Weingarten-Arams research focus includes the use of deliberate simulated practice in improving outcomes in pediatric critical illness, pediatric resuscitation and pediatric airway management. Her other projects involve chronic critical illness, bioethics in pediatric critical care and oxidative injury in respiratory failure. She has been principal investigator and co-investigator on several research projects, and her work has been shared through numerous peer-reviewed publications and invited presentations.</p><p>Dr. Weingarten-Arams is a Diplomate of the National Board of Medical Examiners and the American Board of Pediatrics. She is a Fellow of the American Academy of Pediatrics and the American College of Chest Physicians. She is also a member of several professional organizations, including the American Medical Association, the Pediatric Cardiac Intensive Care Society and the Society of Critical Care Medicine. That same year, she completed an APA-accredited predoctoral internship in clinical psychology-adult neuropsychology track with the Henry Ford Health System. She then went on to complete her postdoctoral fellowship in clinical neuropsychology with the Institute of Living, Hartford Hospital, with focused training in the assessment of memory disorders/dementias, acquired brain injuries, and severe and persistent mental illness.</p><p>Dr. Twaite&rsquo;s research interests include cognitive and emotional functioning following stroke, traumatic brain injury (TBI) and other acquired brain injury in individuals with Parkinson&rsquo;s disease as well as in healthy populations, such as in musicians. Her work has been published in numerous peer-reviewed journals and she has given paper and poster presentations at scientific meetings nationally and internationally. <p>Ivy Tam, MD, FAAP, is an Attending Physician and a Non-neonatal Emergency Transport Coordinator at Children&rsquo;s Hospital at Montefiore. <p>Mark Shlomovich, MD, is Director, Extracorporeal Liver Support and Director, Pediatric Critical Care Transport at Children&rsquo;s Hospital at Montefiore and Assistant Professor, Pediatrics at Montefiore-Einstein. Dr. Shlomovich&rsquo;s clinical focus is on the care of critically ill children with abdominal organ failure, transplantation and post-surgical care. He also focuses on the safe transportation of critically ill children.</p><p>In 2006, Dr. Shlomovich earned his Bachelor of Science in biology from Towson University. He then attended University of Connecticut School of Medicine earning his Doctor of Medicine in 2010. His postgraduate training began at SUNY Downstate Medical Center with a four-year pediatric residency, where he was Chief Pediatric Resident in his final year. He then came to Children's Hospital at Montefiore for a pediatric critical care fellowship, which he completed in 2017.</p><p>Dr. Shlomovich&rsquo;s research interests are in the use of artificial liver support technology, as well as the care of children in liver failure before and after transplantation. In addition, he investigates the safety and efficiency of the transportation of critically ill children. He has shared his research through peer-reviewed journals and abstracts, and is an ad hoc reviewer for BMC Nephrology.</p><p>Dr. Shlomovich is board certified in general pediatrics and pediatric critical care medicine. He is a member of the Society of Critical Care Medicine and the American Academy of Pediatrics.</p>

<p class="MsoNormal">Theresa Serra, MD, joined the CHAM hospitalist division in 2014 after working as a pediatric hospitalist at New York Presbyterian/Lower Manhattan Hospital. Her interests include clinical research and medical education. She has conducted an IRB approved retrospective chart review examining readmission rates in pediatric patients diagnosed with osteomyelitis. Dr. Serra is currently developing a medical Spanish curriculum for faculty members in order to improve communication between patients, families and providers. Along with Courtney McNamara, MD, Dr. Serra is a co-leader of a multidisciplinary committee dedicated to standardizing and improving quality of care for patients hospitalized with the diagnosis of failure to thrive. Dr. Serra is also working on an initiative to identify vaccine delinquency in the inpatient setting through the use of SMART reports.</p> <p class="MsoNormal">Dr. Serra is also collaborating with Lindsey Douglas, MD, MSCR, in an IRB approved quality improvement (QI) project to improve transition-of-care communication between hospitalists and primary care providers and to facilitate the establishment of medical homes. Dr. Serra is also a member of a subcommittee that leads monthly journal clubs/case conferences for pediatric hospitalists.</p>

<p>Raminder Parihar, MD, is Director, Neuromodulation at Montefiore Einstein and Assistant Professor, Neurology at our Albert Einstein College of Medicine. Dr. Parihar evaluates and treats patients with different movement disorders. She is also largely involved in evaluating patients for candidacy for deep brain stimulation (DBS) surgery for the treatment of different movement disorders and initiating and following up stimulation programming.</p><p>After obtaining her Bachelor of Science in biomedical sciences from the City College of New York in 2007, Dr. Parihar completed her Doctor of Medicine at Stony Brook University School of Medicine in 2009. She then came to Albert Einstein College of Medicine, where she completed an internship in medicine in 2010 and her residency in adult neurology in 2013, where she was Chief Resident in her final year. Dr. Parihar completed a fellowship in movement disorders at Harvard/Beth Israel Deaconess Medical Center in 2014.</p><p>Dr. Parihar&rsquo;s research focuses on gait disorders and how they can predict cognitive impairment in patients with Parkinson's disease. She also studies the impact of deep brain stimulation in improving the quality of life in patients with movement disorders. Her work has been published in peer-reviewed book chapters and publications, and she has shared her work through lectures and platform and abstract presentations.</p><p>Dr. Parihar is a Diplomate of the American Board of Psychiatry and Neurology. She is a member of the American Academy of Neurology, the Movement Disorders Society and the American Medical Association (AMA). In 2023, Dr. Parihar was a recipient of the Marquis Who's Who in America Award.</p>
